Read on-are YOU qualified to apply and take the next step in YOUR career?This position leads efforts to identify, educate, and support pregnant women/expecting families with opioid use disorder, seeking to refer them to appropriate resources, including medication assisted treatment (MAT) and integrated behavioral health care and retain them in their programs of care. ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions. 1.Serves as Care Manager for enrolled Meadowlark patients alongside One Health MAT/BH team, and One Health Prenatal team, facilitating warm hand offs from the clinic team to the community PSS or vice versa.2.Assists in the development and deployment of comprehensive community outreach program targeting pregnant women/expecting families with opioid use disorder (OUD).3.Plans, delivers, and maintains records of personal visits, in accordance with Meadowlark and evidence-based program guidelines.4.Facilitates deployment of community resources to support family needs, including connecting families with resource support and MAT/Substance Use Disorder services. 5.Completes required Meadowlark/MAT training, including SBIRT, Motivational Interviewing, and One Health resource support orientation.6.Works closely with One Health's prenatal program, assisting with peer support events, providing follow-up support for primary care visits, conducting educational visits with patients, and connecting prenatal patients with resources. 7.Always maintains professional relationships with clients, adhering strictly to confidentiality guidelines, as identified by HMF and One Health standards. 8.Provides data required for completion of the Meadowlark Quality Assurance/Quality Improvement projects and all required HRSA funding reports.9.Participates in weekly huddles with One Health prenatal and Behavioral Health Teams. 10.Works with delivering hospitals to collect patient-specific data and to assist in discharge of prenatal patients back to their healthcare provider in Big Horn County.11.Maintains a professional demeanor in interactions with clients and community partners.12.Follows home visiting safety guidelines.13.Identifies health literacy needs for patients and provides proper health and parenting education materials.Education/Certificates/Licenses: RequiredDegree in nursing from an accredited nursing programMontana nursing licenseBLS CertificateValid driver's licenseExperience:PreferredTwo to five years? experience in clinical setting of primary care or public healthExperience with MAT and home visitingWe offer a great benefit package including:Generous Health Insurance Benefits403B with 3% company matchPTO packageHoliday PayPay Rate: $30.00 per hour.
