Care Management Assistant

Employment Status: Full-Time (.75 or greater)

Hours per Pay Period: 1.00 = 80 hours (Non-Exempt)

Starting Wage DOE: $17.15 - 21.44

The Case Management Assistant is an integral member of the Care Management team. Works independently with minimal supervision to connect patients with community resources by scheduling appropriate follow-up appointments prior to the hospital discharge. Assists in the delivery of patient care by delivering CMS forms, important messages from Medicare, assists with DME needs, arranges transportation and assists with other needs identified by the care team. Works collaboratively with the care management team, primary care services, pharmacy and other Billings Clinic team members and departments to meet the physical, psychosocial, educational, and spiritual needs of the patient and family.

Essential Job Functions

•Schedules and coordinates requested follow up appointments for patients being discharged from the hospital both with Billings Clinic and non-Billings Clinic providers.

•Advocates for the patient as necessary to ensure timely discharge follow up in the outpatient setting. Has a discussion with patient and family to learn dates and times that will best meet the patient/family needs. Promptly escalates concerns to appropriate chain of command.

•Works to identify patients needing follow up care scheduled by reviewing charts and interacting with Care Managers, hospitalists, IMR physicians, and attending daily rounds.

•Assists patients in addressing barriers to care such as transportation, appointment location, appointment availability and scheduling.

•Follows scheduling protocols for Billings Clinic appointments scheduled to ensure scheduling accuracy.

•Works closely with the inpatient Care Management team and inpatient providers across the hospital to coordinate appointments and address other patient coordination needs prior to discharge.

•Provides the centers of Medicare and Medicaid Services DMS forms (MOON, IMMs) to patients in a safe, timely and effective manner.

•Acts as a Representative for Billings Clinic while interacting with regional facilities including but not limited to physician offices, Indian Health Services, and the Veterans Administration.

•Advocates on behalf of patient, communicating and collaborating with healthcare providers, payers, physicians, and community-based services to assist in establishing an appropriate and integrated care for each patient.

•Actively participates in system and regional process improvement initiatives to improve transitions of care.

•Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is completed in compliance with all clinical guidelines and billing/ reimbursement standards.

Minimum Qualifications

Education

•High School or GED

Experience

•One year of successful experience working in an office, preferred.

•3 to 5 years of successful experience working in, and providing, exceptional customer service, preferred.

• Demonstrated commitment to ongoing education, preferred.
