St. John's Lutheran Ministries C.A.R.E.S. Social Worker - Full-Time in Billings, Montana

Monday Friday 8am 5pm with some evening and weekend hours SUMMARY: The Care Coordinator will follow the Wraparound process to provide care coordination to identified children, youth their families. The C.A.R.E.S. Wraparound model is a strengths-based, family driven diversion program designed to divert youth and families from the child welfare system, juvenile justice system or residential care. Essential Duties and Responsibilities * Facilitate Family Team Conferences and follow the Wraparound process to manage caseload * Build relationships with family members, providers and the community * Promote family voice and choice in work with families and community providers * Practice trauma informed care * Complete all necessary documentation to ensure Wraparound model is followed with fidelity * Perform other incidental or related duties as assigned REQUIREMENTS EDUCATION/EXPERIENCE: * Bachelor’s degree in social work or related field * Knowledge of mental health systems of care and support services in Yellowstone County * 2 years of experience working with children, youth and families that have complex needs and are multi-system involved KNOWLEDGE, SKILLS, AND ABILITIES: * Excellent written and verbal communication interpersonal skills * Demonstrate cultural competence sensitivity to population served * Proven ability to work with diverse groups and build strong partnerships * Support and uphold the mission, beliefs and values of St. John’s United * Proficient in the use of computers and Microsoft office programs and virtual web platforms TRAVEL/OTHER: * Candidate must have valid driver’s license, proof of current insurance and be able to drive throughout the state of MT. Other national travel may be required. * Must be able to pass a thorough background screening upon hire. * Some occasional evening and weekend hours required PHYSICAL DEMANDS: * Requires minimal lifting functions * Requires moderate bending, standing and reaching activities WORKING CONDITIONS: * Normal office setting.
