Hours per Pay Period: 1.00 = 80 hours every two weeks (Exempt)
Starting Wage DOE: $38.96 -$48.71
The Clinical Coordinator assists the Manager and/or Director in providing leadership and management within the Care Management Department. The Clinical Coordinator is responsible and accountable for the coordination of the 7 day a week operational review. Responsibilities include but are not limited to personal management to include clinical and non-clinical personnel selection, orientation/training, scheduling, development/mentoring, performance appraisals, coaching and counseling, disciplinary actions, patient care delivery, physician relationships, intradepartmental and interdepartmental activities, quality control and process improvement activities, regulatory compliance and fiscal control. The Clinical Coordinator assists in the development of policies and procedures. In the absence of the Manager/Director, may assume responsibility and accountability for management of the Department.
Essential Job Functions
• Supports and models behaviors consistent with Billings Clinic’s mission, vision, values, code of business conduct and service expectations. Meets all mandatory organizational and departmental requirements. Maintains competency in all organizational, departmental and outside agency standards as it relates to the environment, employee, patient safety or job performance.
• Provides leadership within Care Management in the accomplishment of organizational/departmental goals and objectives. Performs the functions of interviewing, hiring, scheduling, establishing priorities and assigning workloads, orientation/training, day-to-day timecard management, safety-net review and investigation, performance appraisals, coaching/counseling, disciplinary up to and including discharge as coordinated with the Manager, Director, and Human Resources. Responsible for adhering to established human resource management policies and procedures.
• Provides Leadership for the Care Management staff and assists in the accomplishment of the daily operational needs.
Works collaboratively and effectively with staff, coordinators, manager and director to provide leadership and expert consultation in developing effective care transitions and in meeting the overall needs of the department
Assists with program assessment, analytics, and identification of risks and issues that may compromise the team’s performance and results
• Participates in the development of and assists with implementation of an education plan that addresses individual and departmental needs. Assists and/or conducts regular staff meetings and directs the orientation and training of new staff and provides ongoing education to staff. Creates an environment in which personnel can realize personal growth and development through active participation in departmental decisions, opportunities for professional growth and consistent application of personnel policies.
• Provides a critical role in being the expert consultant on payer billing requirements and provides staff education and development of progresses that support optimal patient care.
• Mentors and develops staff
• Acts as a resource to the Social Services Team and others regarding financial reimbursement issues.
• Plans appropriate staffing practices by maximizing the utilization of resources; forecasts and anticipates the personnel needs of department(s) with regard to fluctuations in the patient workload and productivity standards; maintains staffing at agreed upon budgetary standards.
• Assists Manager/Director with the development, implementation and maintenance of departmental policies, procedures and practices to ensure quality of care and a safe environment for patient/ staff.
• Leads clinical process that provide interventions designed to ensure the coordination and continuity of care and services for at-risk patients with complex chronic medical, social, behavioral or/and mental health illness over time.
• Develops strategies and processes to implement change.
• Effectively leads discussions and presentation as needed.
• Organizes ongoing communication between all health care providers regarding aspects of patient/ care and department(s) functions. Acts to resolve conflicts in a timely manner. Develops collaborative relationships with physicians to enhance quality practice and quality outcomes. Provides consultation for patients/family care related concerns and acts as a patient/family and staff advocate as needed.
• Participates in both short- and long-term planning for departmental functions and budgetary processes and fiscal control.
• Maintains competency in all organizational, departmental and outside agency safety standards relevant to job performance and ensures staff adherence to the same. Monitors and ensures compliance with quality and regulatory standards Prevents, minimizes and/or corrects risks to patients/families, families, physicians, and other health care providers. Investigates unusual occurrences and follows appropriate procedure.
• Utilizes process improvement principles to assess and improve the quality of patient/resident care. Develops quality initiatives as appropriate based on patient satisfaction scores, employee satisfaction scores, regulatory compliance, and/or other issues identified.
• Maintains and displays an appropriate degree of clinical expertise as a Registered Nurse. Acts as a resource for physicians, other health care providers, patients/families, and families. Identifies needs and sets goals for own professional growth and development, meets all mandatory system/departmental educational requirements. Maintains knowledge of current clinical trends and developments as it relates to job scope.
• Maintains competency in all organizational, departmental and outside agency environmental, employee or patient safety standards relevant to job performance.
• Performs other duties as assigned or as needed to meet the needs of the department/organization. Provides back up relief and participates in staffing as needed and/or appropriate.
• Responsible for the oversight of monitoring high risk patients for assigned patient population. System expert for problem solving coordination of care
• Works collaboratively to provide leadership and expert consultation in developing effective care transitions, follow-up process for pending medical treatment and referral results
• Primary liaison with primary care providers, outpatient care managers, coding resources and PFS to ensure compliance with documentation requirements and regulations affecting financial reimbursement for billable outpatient care management services.
• Close working relationship with the primary care services lines and their leadership
• Monitors utilization and practice metrics of assigned patient panels to further refine modes of delivery of care model to maximize clinical, quality and fiscal outcomes. Audits Population health patients’ visits to PCP’s and specialty care.
• Develops and implements systems of care that facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.
• Implements a system of clinical interventions based on risk stratification and evidence based clinical guidelines utilizing various modes of delivery of care models to include telephone communication, primary care site and/or patient home visits
• Incorporates regional strategies in the overall program structure and the alignment of outpatient care manager. Provides • leadership and support to outpatient care managers located at Billings Clinic locations and within the region.
• Performs concurrent and retro medical record reviews evaluating transitions in care and chronic care management documentation
• Required to complete 4 hours of annual training per year for advanced orthopedic and spine certification requirements.

Minimum Qualifications
Education
• Bachelor’s degree in nursing
Experience
• Four (4) years care management experience
Certifications and Licenses
• Current Montana state license as Registered Nurse
Or an equivalent combination of education and experience relating to the above tasks, knowledge, skills and abilities will be considered. Employees that require a licensed or certification must be properly licensed/certified and the licensure/certification must be in good standing.
