This position is located in the Gallatin City-County Health Department. Under general direction of the Maternal Child Health Program Manager and the Human Services Director, carries out initial needs assessment for professional home visitation services for local families. The incumbent will work with primary care providers, families, and partner organizations to enroll clients into appropriate evidence-based public health home visitation programs, including the Healthy Gallatin Home Visiting Program. The Home Visitor will participate the development of supportive

resources for social and emotional wellbeing of children, establish referral partnerships with primary and prenatal care providers, and implement home visiting services that will identify, assess, and connect families with appropriate services, with an emphasis on mental health and family support services. This position requires home visits to families that may involve contentious situations and exposure to the risk of contagious diseases. This position reports to the Maternal Child Health Program Manager and Human Services Director and does not supervise other County personnel. This position is grant funded and is contingent on future funding.

MINIMUM QUALIFICATIONS: This work requires knowledge of the principles and practices of maternal child health home visiting; family and community assessments; human growth and development; educational methods for individuals, groups and the public in general; public health laws and rules; community resources; program planning and implementation; record keeping and basic computer skills. Also required is the ability to communicate effectively verbally and in writing. The incumbent must develop knowledge of all of the Department’s programs in order to provide assistance as needed.

EDUCATION/EXPERIENCE: The required knowledge, skills and abilities are typically acquired through a combination of education and experience equivalent to a bachelor’s degree in a related filed such as social work or community health and three (3) to four (4) years of work experience demonstrating progressive independence in assigned responsibilities.

PREFERRED QUALIFICATIONS:Public Health experience in home visiting preferred but not required. Spanish speaking bilingual preferred but not required. Past experience administering or designing community-based public health interventions preferred but not required.

LICENSES: Applicants must possess a valid driver’s license, provide transportation to conduct out of office job duties (mileage reimbursement provided under County policy), and be insurable under the County’s liability policies.

As a condition of hire, the final candidate will be required to successfully pass a criminal history check and background investigation.
