Provides assistance and medically necessary support services to terminally ill patients in the home or other community setting. Maintains medical equipment and the equipment rooms. Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines.
Adheres to certified nursing assistant standards as defined by the State of Montana.
Perform personal care tasks as assigned by individual plans of care.
Duties include but are not limited to: bathing, dressing, medication reminders, ambulation assistance, and escort to medical appointments, maintenance of a safe and healthy living environment, errand running, preparation of meals, recording vital signs, weighing patients, nail and skin care, toileting, oral care, changing linens, and other assignments as may be appropriate.
Notifies the primary hospice nurse and applicable team members of any patient who has experienced significant distress, change in status or death during the work shift.
Maintains a current and complete record in order to have pertinent background information on each hospice patient.
Adjusts constructively and flexibly to meet the requirements of different work situations, including after hour and weekend on-call coverage as needed.
Current CNA certification in the State of Montana, good standing.
Knowledge of elderly and disabled helpful.
Willing to complete Home Health Aide training (HHA).
Must have the ability to respond calmly and effectively in stressful situations and communicate effectively with other health care providers.
Strong, effective written and verbal communication and interpersonal skills.
Must possess strong organizational and time management skills
Ability to move independently or with reasonable accommodation within the facility and community.
Applicant must be physically and mentally able to perform all requirements of the job.
Current driver’s license and reliable transportation.
Applicant must pass a background and DMV check.
CNA certification mandatory with willingness to become HHA certified.
0-2 year experience working with the elderly and disabled.
