AWARE seeks to provide opportunities for employees to grow professionally through its tuition advancement program, professional development opportunities and in-house trainings.Principal Duties & Responsibilities:Obtain vital health information for residents in group homes and document in the electronic health record.Work with existing protocols and personnel to train and assist residential staff in collecting necessary medical information. Perform similar role in outpatient practice setting. Perform and/or oversee health screening processes as appropriate to different settings within AWARE, including for general health ROS, suicide risk, medication side effects including Abnormal Involuntary movements (AIMs).Provide leadership and expertise to group home staff in caring for residents with chronic medical conditions, e.g diabetes, asthma, arthritis or other musculoskeletal disorders.Provide back-up to other nurses in AWARE for vacations and emergencies, or special-need situations. This may include patients with more extensive mobility or other medical issues, administration of injectable medication. Travel to group home or clinic locations in central-western Montana one or more times per week, and/or work with staff and patients via telehealth.
