CASCADE COUNTY Parents as Teachers Program Manager in GREAT FALLS, Montana

Education/ Experience/ Training: - Bachelor's Degree in Nursing, Social Work or equivalent and two (2) years of experience in nursing or home visiting and four (4) years of experience providing PAT services to ?at-risk? families. Certifications: - Parents as Teacher Model certified in good standing. - Valid Driver's License issued by the State of Montana. *POSITION DESCRIPTION The Parents as Teachers (PAT) Program Manager (Manager) at the City-County Health Department (CCHD) works in the Family Health Services Division and is responsible for the oversight of the PAT Home Visitation program to ensure compliance with the individual model requirements, assessment and evaluation of the work performance of PAT Home Visitors staff. The Manager develops corrective action plans with staff to address performance deficiencies and provides mentoring, supports, guidance and training to and for PAT Home Visitors staff. This position is responsible for the preparation and submittal of Maternal, Infant and Early Childhood Home Visiting (MIECHV) Program and PAT data reports and identifying training needs for PAT home visiting staff. The Manager may carry a PAT caseload and perform home visits, developmental screening activities and facilitate parenting classes to at risk families by using a strength-based approach with all families while challenging them to find effective approaches and solutions for issues related to parenting and child development and provide families with information about available community resources and services and assists parents in developing and fostering healthy self-concepts, self-help skills and self-reliance.. This position requires active leadership involvement with Prevent Child Abuse and Neglect coalition (PCAN), the Home Visiting Advisory Committee and local coalitions to assist with the Cascade County Community Health Needs Assessment process under the United Way Healthy Lives Vibrant Futures Initiative. This includes facilitating and attending coalition and advisory council meetings; promoting collaboration with CCHD programs; developing, and updating websites; assisting in collecting early childhood related community data; developing, implementing, and evaluating PCAN's strategic plan, grant writing and reporting; analyzing health indicators and working collaboratively on developing health policies related to child abuse and neglect. The Manager develops marketing materials and media materials, campaigns and opportunities with the primary focus of promoting CCHD early intervention programs and strengthening community partner relationships, and performs other duties as required or assigned. *ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Assists the Division Manager oversight of evidence-based PAT home visitation model essential requirements and makes policy development recommendations related to the PAT program; Provides Reflective Supervision to PAT Home Visitors as required by PAT model developers during absence of the Division Manager; Provides model support, guidance and assistance with home visiting training; Provides onboarding of new PAT home visitors; Assesses and evaluates PAT Home Visitors staff and provide such assessment and evaluation to the Division Manager; Participates with the Division Manager in hiring and, if necessary, discipline/corrective action process with PAT Home Visitors; Participates as a productive and contributing member of the CCHD Family Health Services Leadership Team; Participates in model and funding required conference calls; Assists staff with support of and navigating the MIECHV data system as needed, coordinates and provides training if necessary; Assures self and staff are accurately entering data within the required time frames; Prepares and submits monthly data reports and assists with quarterly and yearly PAT model reporting requirements; Assists with home visiting program planning and provides home visit monitoring for program staff; Assures PAT grou meetings are held according to PAT guidelines; Facilitates and coordinates monthly working group coalition and advisory board meetings and quarterly steering committee meetings; Attends monthly meetings with the Montana Early Childhood Coalition; Participates in the quarterly Healthy Lives Vibrant Futures Steering Committee; Recruits and maintains a diverse coalition membership with community and state partners; Coalition strategic plan development and implementation; Grant writing and reporting; Develops marketing and media materials, and campaigns to promote CCHD's early intervention programs and to prevent child abuse and neglect; Serves as a liaison among medical and social service providers in the community and acts as an advocate for families; Creates and maintains respectful partnerships with clients and home visiting staff; Demonstrates ability to assume primary responsibility for planning and implementing of home visits and activities with parents of young children, prenatally through kindergarten; Drives to families' homes and delivers health, educational, nutritional and child growth and development information to caregivers following PAT guidelines; Assures home visiting staff is delivering all model components as required; Completes developmental screening and all other model required assessments with families within the PAT expected timelines and in accordance with program guidelines; Assists and supports families in locating resources and scheduling appointments with community resources that best meet their needs and ensures seamless quality services for all enrolled pregnant women, children and their families; Reviews each child's/family's goals and documents individual plans designed to promote current learning and attends to nutritional, health and social issues; Maintains accurate written/electronic records including health information, assessments, screening instruments, anecdotal observations and other required forms; Attends and participates in required model trainings and fulfills the minimum amount of yearly professional development hours; Maintains assigned caseloads and conducts home visits with families in accordance to program guidelines; Works as a team player with staff, parents and other community organizations to ensure quality services and program requirements are met to carry out the goals and objectives of PAT; Maintains family records in accordance with agency and funding source requirements and enters PAT information into MIECHV data system as required; Reports all suspected child abuse and neglect as required by law and documents accordingly; Informs supervisor of all possible reports to the Child and Family Services Division of the Department of Public Health and Human Services; Follows CCHD and County policies and procedures; Complies with and strictly adheres to all CCHD, Health Insurance Portability and Accountability Act (HIPAA), County, state and federal privacy and security laws, rules, policies, guidelines and practices governing and controlling confidentiality and data security with regard to the handling and maintenance of sensitive client information and protected health information; Establishes and maintains productive working relationships with fellow employees, supervisors, and the public; Actively seeks and utilizes the most current program specific information; Performs other duties as required or assigned. **Knowledge and understanding of: PAT program essential requirements;
