POSITION OVERVIEW

Consumer Direct Care Network Montana is seeking a self-starting, energetic and people-oriented Service Coordinator to work in a dynamic and fast-paced environment located in our Great Falls, MT office. In this role you will maintain and coordinate a caseload of clients enrolled in Medicaid programs, coordinate clients’ in-home care needs in compliance with Federal/State Regulations and Managed Care Organization (MCO) program rules. You will also assist with daily intake and referral processes to ensure positive business growth and development. A successful candidate will have strong interpersonal abilities, provide exceptional customer service, and have a high value for teamwork.

WHAT YOU WILL DO

  • Enroll clients and employees in services

  • Conduct necessary client assessments and home visits

  • Communicate effectively with referral sources, clients and employees

  • Conduct business in accordance with state, federal, MCO and contract requirements

  • Maintain necessary skills and knowledge to coordinate work flow

  • Participate in professional development and training activities

  • Collaborate with stakeholders regarding delivery of services

  • Represent company at stakeholder meetings, health fairs and provider fairs

  • Market services to referral sources

  • All other duties as assigned

QUALIFICATIONS

  • High school or GED required

  • 1-year working experience in an in-home caregiving role

  • Great time management skills to help successfully meet deadlines

  • Proficient in working on a computer – specifically MS Office Suite, Adobe Acrobat, and Skype for Business

  • Self-motivated worker with effective multi-tasking skills

  • Valid driver’s license and reliable transportation

  • Must be able to successfully pass a background screening
