Transition Services - Lead Coordinator
ESGW Adult Services Transition Services, support adults in Great Falls with developmental/intellectual disabilities and other significant barriers who live on their own. We hire people who are customer focused, emotionally mature, caring, innovative and have a strong desire to help people in need of support. The Lead Coordinator is responsible for positive, hands-on oversight, and daily operations, maintaining a respectful and supportive environment for people served, staff and stakeholders. Is a resource for staff on a daily basis to ensure quality programming for each individual served. Responsible for assisting the Manager in the hiring, training and evaluation of staff. Ensures daily schedules meet the needs of people served and provides support and follow-up for incidents and accidents involving program staff or people served. Ensures the programs and supports of people served are being provided, reviewed, and documented in a timely manner according to the PSP.
One of the primary objectives of the programs offered is to promote independence, productivity and inclusion in the community that improves the quality of life for every person we serve. You will be responsible for working with people with a variety of developmental disabilities and acquired brain injuries as they work to achieve their highest level of independence. The primary responsibility for this position is to provide direct supports to individuals participating in services to include transportation, vocational, activities of daily living and health/safety. This position requires superior customer service skills, the ability to effectively handle competing priorities, the ability to interface with and assess individual needs provide positive reinforcement to a wide variety of customers with varying skill level, be creative and believe in the mission of ESGW. Must have superior organizational skills
Program development, behavioral support and leadership skills required.
Must be self-motivated, flexible, enjoy challenges and must be willing to work as part of a team.
Must be able to learn and utilize computer programs such as Therap, Red Carpet, GroupWise etc.
Ability to move independently or with reasonable accommodation within the facility and community.
Applicant must be physically and mentally able to perform all requirements of the job.
Applicant must pass a background check and DMV check.
Associate's Degree or equivalent
3-5 Years in DD services or related social service or health care
