PUREVIEW HEALTH CENTER Case Manager - Social Worker in HELENA, Montana

1.General Statement Under the general supervision of the Behavioral Health Program Director, this position provides health education, health, social and environmental assessments and case management services to individual clients and families. 2.Duties and Responsibilities Provides case management services to individual clients and familiesPerforms health, social, and environmental screenings and assessmentsProvides health educationDevelops case management plans to address health, behavioral and other care related issuesAssists in community outreach Provides care coordination and wrap around services for programs like Ryan White HIV/AIDS Program, Medication Assisted Treatment and Hepatitis C careAssists in data entry, tracking and reporting of required program utilizationProvides client support, advocacy and linkage to community support services and organizationsCoordinates services with community service agenciesMonitors individuals and families progress towards achieving plan goalsMaintains client records and documentationReports instances of abuse and neglectPrepares reports on case management activitiesProvides crisis assistance as necessary and resources permit Participates, as needed, in the PureView Health Centers (PVHC) performance improvement activitiesPerforms other duties as assigned3.Required Qualifications ? Experience and Education The above knowledge, skills, and abilities are typically acquired through a combination of education and experience equivalent to:Four (4) year degree in Counseling, Social Work, or related fieldOne (1) year experience in case managementHealth clinic experience preferredExperience providing outreach to vulnerable populations preferred4.Knowledge Skills and Abilities Montana Code Annotated PureView Health Center policies and proceduresElectronic medical records Patient Centered Medical Home practicesHealth and welfare laws and ordinances; Public abuse reporting lawsProvisions of Americans with Disabilities ActMedical clinic practices and principlesOrganize resourcesEstablish prioritiesPrepare reports according to prescribed standardsEnter electronic medical recordsDevelop and evaluate treatment options and strategiesListen and understand information presented through spoken wordsObserve, receive and obtain information from relevant sourcesUnderstand and follows oral and/or written policies, procedures, and instructionsOperate computer systems and related softwareCommunicate effectively orally and in writingMake formal presentationsEstablish effective working relationships with patients, fellow employees, supervisors, community service organizations/ representatives, and citizens5.Special RequirementsNone6.Physical Demands Duties generally performed, in a medical clinic environment, where hazards and discomforts are controlled and modifiable. This position requires the ability to stand, bend at the waist, kneel, reach over the head, talk, hear, and see. Must be able to move or lift documents and materials weighing up to 30 pounds. Position requires knowledge and use of typical office equipment including telephone, copiers, fax machines, and personal computer. Position requires frequent contact with clinic employees and clients, and occasional contact with community service agencies and citizens.
