St. Peters Health Hospice Certified Home Health Aide - Days (1.0 FTE) in Helena, Montana

Hospice Cer tified home health nursing aide's (CHHA) main role is to provide direct patient care in the home or setting where the patient refers to as home. Hospice Certified home health aides perform basic duties such as bathing, assistance with activities of daily living, safety and vital signs. The CHHA must demonstrate competency with all core skills as required by the Certified Nursing Assistant licensure. Demonstrates effective verbal and written communication skills. Understands basic knowledge of ethical issues and is able to provide care to terminally ill patients. Provides caring holistic person-centered care. Demonstrates knowledge of basic infection control practices, such as hand washing technique. The CHHA is under supervision of the Hospice clinical coordinator/supervisor and/or director. Ability to communicate with the Hospice RN case manager for concerns that may arise in the home while performing patient care. On a daily basis communicate with the CHHA scheduler. The CHHA follows an individualized plan of care that is developed by the RN case manager. The CHHA will demonstrate an understanding of the plan of care prior to the first visit with patient. Maintains HIPPA compliance. Has a general understanding of regulatory compliance to assure the provision of safe, quality care.

COMMUNICATION: Demonstrates good communication skills to facilitate coordination of care with the Hospice RN case manager and CHHA scheduler. Attends meetings and morning huddles as needed to encourage regular communication with the interdisciplinary team. Maintains open communication with the interdisciplinary team. Maintains open communication with the Director and/or Hospice clinical coordinator/supervisor in relation to patient concerns. Utilizes appropriate chain of command to collaboratively and proactively problem solve as needed. Reports any deviations of the plan of care to the RN case manager. Attends education when offered related to competencies or changes in processes.

ADAPTABILITY: Has a “can do” attitude, is an active team member, and a no-blaming environment while setting the tone for the department by being a good role model. Maintains a positive attitude and flexibility regarding fluctuating patient census and staffing needs. Demonstrates strong communication skills and actively completes assignments, documentation per agency policy. Able to prioritize and multi-task when the department gets busy. Keeps calm during stressful times.
