Kalispell Regional Healthcare Ambulatory Clinical Pharmacist in Kalispell, Montana

The Ambulatory Clinical Pharmacist will demonstrate the ability to deliver appropriate and safe medical care in an efficient and cost effective manner, among a team of multidisciplinary health care professionals. The scope of practice includes direct patient care though collaborative practice agreements, patient education, medication reconciliation, and drug information.

Responsibilities

  1. *Evaluates and verifies orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy. Clarifies physicians’ orders as necessary through direct and timely contact with prescriber or designated practitioner.

  2. *Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff.

  3. *Performs daily review of pharmacy reports including pharmacokinetic monitoring and other drug or patient specific reports as required.

  4. *Documents clinical interventions appropriately in electronic medical record.

  5. *Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.

  6. *Reviews and updates Collaborative Practice Agreements annually or as required by leadership.

  7. *Supports and precepts pharmacy students on Ambulatory Care practice rotations.

  8. *Assesses and evaluates the medical and drug literature. Compares and contrasts therapeutic regimens and uses for new or existing drug treatments.

  9. *Provides direct and indirect patient care for patients through Collaborative Practice Agreements. These duties include, but may not be limited to: monitoring, adjustment of medications, drug/drug interactions, drug/disease state interactions, lab interpretation and basic physical assessment skills.

  10. *Develops and participates in operational and clinical projects as determined by current departmental needs and ongoing projects.

  11. *Performs direct patient care rounding, order verification, discharge counseling, and education of other healthcare professionals.

Qualifications

  1. PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy, or equivalent clinical experience

  2. Current Montana State Board of Pharmacy, Registered Pharmacist License required.

  3. Experience as an Ambulatory Care Pharmacist preferred.

  4. Completion of Pharmacy Residency Program or Board Certified Pharmacotherapy Specialist (BCPS) preferred.

  5. Demonstrated ability to make independent judgments under stressful situations at times. Must work effectively and efficiently without direct supervision.

  6. Excellent communication skills, both verbal and written, required.

  7. Must possess accurate data entry skills, analytical and advanced math skills.
