Logan Health Licensed School Based Therapist - LCSW or LCPC (Relocation assistance available) in Kalispell, Montana

Job Description Summary:

This Clinic position is school based providing Outpatient Therapy services for students, staff and family members of each. Service includes Individual, Group, Family Therapy, Screening Assessment and Brief Therapy, Care Coordination, and Prevention Education provided at school site. Candidate must be flexible with scheduling and have ability to provide service at more than one school site on a regular basis if necessary. Experience with Telehealth is helpful. Strong clinical and collaborative skills needed. Organized and able to work independently while maintaining HIPAA compliance and Logan Health Standards of Care. Proficient with use of technology and able to adapt to changes with Information Technology.

Relocate to Montana for this position and receive up to $12,500. This job is eligible for the CARE in Montana Program, which reimburses you for relocation expenses.   • Master’s Degree in Social Work or related counseling field from an accredited university required.

  • Current Montana LCSW or LCPC licensure.

  • Experience working collaboratively with families, physicians, and agencies. Must have the ability and skills in establishing and maintaining working relationships with families, coworkers, and other professionals and all collaborative partners in the community.

  • Excellent communication skills, both verbal and written, required.

  • Minimum of two (2) years of experience in mental health treatment programs or substance abuse.

  • Knowledge of evidenced based practice and experience in assessment, crisis assessment, brief therapies, and clinical case management.

  • Previous experience with electronic medical record (EMR).

Job Duties:

  • *Triages referrals from providers. Ensures on-going liaison occurs with patients, medical providers, and families.

  • *Performs initial and comprehensive assessments, including diagnostic evaluation, risk assessment, behavioral systems, level of care need, and other information necessary to determine treatment.

  • *Performs as part of a multi-disciplinary treatment team. Provides consultation and education services.

  • *Coordinates patient transitions into services and discharges from treatment.

  • *Completes progress notes and documentation in EMR for all services provided to patient.

  • *Provides brief individual, group, and family therapy, clinical case management, and outreach services in an effort to fulfill the patient’s treatment goals, wellness and recovery.

  • *Participates in development of individual and/or group treatment plans utilizing a variety of brief psychotherapeutic, psychosocial and psychoeducational modalities.

  • *Provides crisis management as needed.

  • *Performs as a consultant and resource in integration development, guiding group decision making, implementing plans, and improving quality of care.

  • *Adapts interventions to the work flow and pace to that of primary care, including rapid assessment, brief treatment, and a high daily volume of contact.
