Job Description Summary:
Logan Health Home Health is looking for an LPN to join its compassionate team serving the Flathead Valley. If you love going out in to the community to meet your patients where they are, this may be the position for you!
This position provides practical nursing care to patients within the scope of practice and as applicable to Home Health.
Qualifications for
Current Montana Licensed Practical Nurse ( LPN ) license required .
Current BLS certification required as applicable to assigned area(s) .
Previous experience in home care or hospice preferred .
Must have capability to connect to internet for daily data exchange.
To be eligible for the Driver portion of the position, Current Driver’s License required .
Driving record for the previous thirty-six (36) months must not reflect any major driving offenses ( i.e ., DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).
In addition, personal vehicles used for LH business: State law minimum liability coverage limits required .
Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Commitment to working in a team environment and maintaining confidentiality as needed.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy .
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed .
Job Specific Duties:
Provides skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in home care/hospice
Exhibits a commitment to professionalism, civility, teamwork, and customer service while undertaking all tasks and interactions with others .
Exhibits knowledge of policies and procedures as they relate to the specific job duties, patient care and safety .
Exhibits knowledge of staffing standards and a demonstrated commitment to cost containment with all resources .
Operates and maintains equipment to departmental standards .
Maintains nursing standards of care and executes appropriate provider orders. While executing patient care, exhibits knowledge of and maintains the patient’s right to diversity, privacy, and confidentiality.
Performs a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.
Administers medications, treatment, and provider - specific care to patients in compliance with n ursing policies/procedures , and LPN scope of practice .
Communicates all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner and as defined by policies and pro cedures and/or provider orders .
Documents all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements .
Contributes to maintaining an updated, individualized plan of care that includes goals, interventions, medications and visit frequency orders .
Maintains a thorough knowledge of infection control procedures and strictly adhere s to policies and procedures .

Day Shift - 8 Hours (United States of America)
Schedule Details:
Please know schedules and shifts are subject to change based on patient care and department/organizational needs.
Schedule: 40 hours per week, 8 hour day shifts
Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
