Logan Health LPN - Hospice Care Coordinator - PRN in Kalispell, Montana

Job Description Summary:

Logan Health Hospice is looking for an LPN to join their team as a Care Coordinator. This position may also assist our Home Health team with coverage on occasion.

*12% PRN differential pay

Per assigned area(s):

• This position coordinates the day-to-day schedule optimization, training and staffing for the assigned area(s).

• This position coordinates patient care and communicates with clinicians regarding documentation to obtain an accurate and complete clinical record. Ensures timely referral and admission process, transitional care coordination, comprehensive admission assessment and development of the Plan of Care to address assessed needs. Complies with Logan Health’s compliance program initiatives.

Qualifications:

  • Current Montana Licensed Practical Nurse (LPN) license required.

  • Minimum of two (2) years’ LPN experience required.

  • CPR certification required as applicable to assigned area(s).

  • Knowledge of relevant equipment, procedures, strategies, and clinical skills to ensure optimal patient care preferred.

  • Minimum of one (1) year experience in a role exhibiting strong collaborative and coordination skills preferred.

  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Exhibits knowledge of policies and procedures as they relate to the job duties, patient care and safety. Exhibits knowledge of staffing standards and a demonstrated commitment to cost containment with all resources.

  • Reviews assigned area(s) staff schedule and works in collaboration with stakeholders to ensure adequate coverage as applicable to assigned area(s).

  • Assists leadership with maintaining accountability to policies and procedures.

  • Completes and maintains staff schedule and call schedule in conjunction with assigned leadership as applicable to assigned area(s).

  • As applicable to assigned area(s); Collaborates with vendors to maintain equipment in working order, troubleshoots and follows-up with any issue. Coordinates repairs and service as needed.

  • Exhibits skill while utilizing various equipment in the assigned area(s). Utilizes appropriate care of equipment and maintains skills in cleaning and storing of all scopes in the assigned area(s).

  • Demonstrates a commitment to professionalism while undertaking and completing all competencies and advance primary certification as applicable to assigned area(s).

  • Transports patients and/or assists with transporting as needed as applicable to assigned area(s).

  • Performs auditing activities throughout assigned area(s). Utilizes Office of Inspector General (OIG) work plan, summarizes and presents audit findings to leadership and clinicians as applicable to assigned area(s).

  • Coordinates and performs referral and intake processes, triages patients and schedules admissions based on patient need, as applicable to assigned area(s).

  • Coordinates and communicates scheduling of field clinicians in planning and providing care as applicable to assigned area(s).

  • As applicable to assigned area(s); Supports a smooth transition of care for patients from one level of care to another. Provides acute care, skilled nursing facility, and emergency department (ED) follow up. Identifies high-risk patients who would benefit from chronic care management and works collaboratively with the primary care provider, RN navigator, patient, and family to get patient enrolled in chronic care management. Reviews records and ensures documentation requirements are complete prior to submitting to payers, per department protocol.

  • As applicable to assigned area(s); Audits the medical records of patients admitted to assigned area(s) per department protocol. Ensures admission documentation and Plan of Care identifies interventions and treatments required to meet the patient’s needs. Communicates deficiencies to appropriate clinician.

  • Coordinates the HHAs and CNA's schedules and assignments as applicable to assigned area(s).

  • Maintains confidentiality and compliance with HIPAA standards.

  • Coordinates and obtains VA authorization and recertification as applicable to assigned area(s).

  • As applicable to assigned area(s); Performs clinical nursing visits as needed. Builds therapeutic and trusting relationships with patients, families, and caregivers through effective communication and listening skills. Utilizes motivational interviewing techniques to assist patients in meeting goals and assisting with chronic disease management.

  • As applicable to assigned area(s); Provides basic assessments. Assesses patient needs upon initial encounter and periodically throughout navigation. Matches unmet needs with appropriate services, referrals, and support services, such as dietitians, providers, social work, pharmacy, and financial services. Acts as a liaison between the patients, families, caregivers, and the providers to optimize patient outcomes.

Schedule:

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
