Job Description Summary:
We have an opportunity for a full-time night shift LPN to join our Transitional Care Unit team at Brendan House Skilled Nursing!
• $7,500 Sign-on Bonus!
• Night Shift Differential pay of $3.75 per hour
• Weekend Shift Differential pay of $2.25 per hour

This position provides practical nursing care to patients within the scope of practice and as applicable to assigned area(s).
Qualifications:
Current Montana Licensed Practical Nurse (LPN) license required.
Current BLS certification required as applicable to assigned area(s).
Previous experience in a similar position preferred.
Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Commitment to working in a team environment and maintaining confidentiality as needed.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.
Job Specific Duties:
Provides skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in the assigned area(s).
Exhibits a commitment to professionalism, civility, teamwork, and customer service while undertaking all tasks and interactions with others.
Exhibits knowledge of policies and procedures as they relate to the specific job duties, patient care and safety and as applicable to assigned area(s).
Exhibits knowledge of staffing standards and a demonstrated commitment to cost containment with all resources.
Operates and maintains equipment to departmental standards as applicable to assigned area(s).
Maintains nursing standards of care and executes appropriate provider orders. While executing patient care, exhibits knowledge of and maintains the patient’s right to diversity, privacy, and confidentiality.
As applicable to assigned area(s): Performs a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.
Administers medications, treatment, and provider-specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s).
Communicates all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner and as defined by policies and procedures and/or provider orders as applicable to assigned area(s).
Documents all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements as applicable to assigned area(s).
Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders as applicable to assigned area(s).
Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.

Schedule Details:
Please know schedules and shifts are subject to change based on patient care and department/organizational needs.
Schedule: Night Shift, 36 hours per week (12-hour shifts)
Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
