Kalispell Regional Healthcare Population Health Data Abstractor in Kalispell, Montana

Overview

This position seeks to improve the health of patient populations, improve patient satisfaction, engagement, and strives to decrease overall healthcare costs. Improves quality of care by linking at risk or high risk patients to needed care using payer reports, health information technology (HIT) and EMR tools and registries to identify patient and population gaps in care.

Responsibilities

  1. *Supports clinical improvement, regulatory reporting, and collaborates with payer partners to support the initiatives with-in the department of Population Health and the primary care and specialty clinics.

  2. *Abstracts clinical quality data utilizing multiple electronic medical record systems in accordance with established quality reporting guidelines.

  3. *Reviews patient registries and data sets to identify gaps in care, disease specific screenings, and basic preventative health screens. Communicates appropriately in an effort to ensure timely closure of identified gaps.

  4. *Collects and tracks process improvement and quality measure data per department protocol. Conducts analysis of data in collaboration with stakeholders to assist in identification of improvement activities. Presents data collected in an organized format to facilitate analysis and identification of improvement opportunities.

  5. *Follows up and assists clinics in addressing overdue referrals and orders as they pertain to population health quality measures.

  6. *Assists patients with scheduling to address any gaps in care that may be needed for chronic disease management per protocol. Assists with chart prep as assigned.

  7. *Reviews payer/provider patient attributions to ensure capture and proper management of attributed lives from contracted population health agreements and initiative. Collaborates and shares data with stakeholders to identify high risk attributions for care management needs and referrals.

  8. *Monitors patient to provider attributions, ensures accuracy and methodology for attribution is applied, and reviews in an ongoing manner to improve continuity and empanelment initiatives and goals.

  9. *Efficiently and directly collaborates with clinics and providers in multiple locations in order to meet population health standards per department protocol.

  10. *Provides excellent customer service, listens skillfully and displays a willingness and ability to acknowledge the needs, expectations and values of others.

Other duties may be assigned similar to the above consistent with knowledge, skills and abilities required for the job. Not all of the duties may be assigned to a position.

Qualifications

  1. Minimum of two (2) years’ comparable healthcare setting experience required. Primary care experience preferred.

  2. Experience and knowledge of electronic health records required.

  3. Current Certified Medical Assistant (CMA), Registered Medical Assistant (RMA) or Licensed Practical Nurse (LPN) preferred.

  4. Knowledge of medical terminology and CPT and ICD-10 codes preferred.

  5. Must possess and maintain computer skills to include an intermediate knowledge of Word, Outlook, Excel, and the ability to learn other software as needed. Experience with data analytics and quality metrics preferred.

  6. Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  7. Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

  8. Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.Commitment to working in a team environment and maintaining confidentiality as needed..
