Job Description

Purpose

To facilitate the day to day operations on the floor in planning, organizing, directing and coordinating guest/resident care.

  • Plan work schedules and assign work to staff to ensure adequate service and coverage.

  • Assist the Director of Nursing in the follow through of annual departmental operations and budgets.

    Job Specific Duties

  • Exhibit a demonstrated commitment to the mission and Immanuel Spirit of Immanuel Living.

  • Work as a facilitator with the goal to have the floor function as efficiently as possible to benefit the guests/residents.

  • The role changes daily based on the needs of the floor.

  • Create an environment of collaboration and mentorship.

  • Support customer service guidelines by developing patient/resident and family relations, supporting the care team to provide exemplary customer service and demonstrating proactive service recovery.

  • Work the floor if there is a licensed nurse opening that has not been filled.

  • Implement resident/guest plan of care or assist other licensed personnel in implementing the plan of care

  • Round units every morning, provide nursing report to the IDT team in clinical meeting.

  • Assist with admissions throughout the day such as room ready, equipment needed in room, pharmacy notification, orders in PCC.

  • Assist with discharges and education of guest/resident as needed by the nursing team.

  • Monitor unit dashboard and follow up on incomplete UDA’s, charting MAR/TAR/POC.

  • Assists with treatments ie, catheter changes, IV starts, dressing changes, immunizations etc.

  • Draw blood on Monday and Wednesday morning and take it to lab if needed.

  • Complete/assist with wound rounds on Wednesday.

  • Assist in dining rooms with meals, passing trays/feeding residents/guests.

  • Complete follow up by the end of the day.

  • Review provider folders daily, and round with provider as needed.

    Schedule

  • Works Three 12 hour shifts one week, works Four 12 hour shifts the following week. Rotating to a different unit every 2 months.

Requirements

Education and Experience

  • Current unencumbered Montana RN/LPN License.

  • Current Healthcare Provider CPR certification.

  • A minimum of 2 years of experience in long term care/skilled nursing. Will consider combinations of experience and education.

  • Possess knowledge of the health care environment, long term care regulations, and care delivery systems as they apply to the clinical practices on the unit.

  • Ability to employ critical thinking skills, decision making and professional judgment.

    Essential Physical and Mental Requirements

  • The incumbent must be at least 18 years of age, as position operates potentially hazardous equipment and handles and dispenses prescribed medication.

  • The incumbent must be able to perform multiple tasks simultaneously and work under time and/or deadline pressures.

  • Required vision abilities include close up and distance, peripheral vision, depth perception, and ability to adjust focus.
