WESTERN MONTANA MENTAL HEALTH CENTER MACT-Care Coordinator in LIBBY, Montana
The MACT Care Coordinator is a transdisciplinary team member within a fast-paced and energized Assertive Community Treatment team giving clients a team wrap-around-care approach. Supporting our community, working one-on-one with clients to remove barriers and celebrate victories, all while focusing on stabilization and integration for each client. Care Coordinators organize and monitor client treatment, connect clients with community resources, and complete treatment plans and documentation. The MACT Care Coordinator has excellent communication, team player, critical thinking and problem solving skills. Able to utilize ethical reflection, and has the ability to complete treatment plans and documentation.
