POSITION SUMMARY: The RN will be clinically competent while delivering and promoting physical and mental health care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. The RN will effectively interact with patients, significant others and other health team members while maintaining all standards of professional nursing.

The LPN works to ensure that the outpatient clinical team functions at optimal efficiency so care is delivered in a holistic, patient-centered way that advances quality outcomes for individuals and the community. Working with patient care teams, the LPN performs care management, direct clinical nursing, laboratory services, and monitoring of performance data to advance the organizational mission.

COMPENSATION: LPN ($19.85-$21.21 hr/ DOE + $2,500 retention bonus) RN ($30.40-$32.38 hr/ DOE + $5,000 retention bonus)

EMPLOYMENT TYPE: Full Time

AVAILABILITY: 4/10 schedule

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  • Triage patients, collecting vitals, review of systems, health history, and medication reconciliation, PHQ-2 and PHQ-9, GAD 7 information.

  • Documents data into Electronic Health Record in accordance with established Patient Centered Medical Home (PCMH), Uniform Data System (UDS) and Meaningful use measures.

  • Provides basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.

  • Administers medications per provider orders.

  • Anticipates and prepares examination room with equipment and supplies required for the visit.

  • Assists provider with examination and procedures per request.

  • Performs lab testing including quality controls, maintains all laboratory/diagnostic test logs, and reports all critical lab values immediately.

  • Administers vaccines to patients in accordance with current vaccine schedules.

  • Participates in huddle with primary care team.

  • Handles telephone encounters and medication refill requests for primary care team.

  • Effectively uses the electronic medical record for charting patient nursing visits and telephone contacts.

  • Keeps client medical records updated for reviews, audits, and contact purposes.

  • Participates in quality assurance/quality improvement projects as needed or requested.

  • Works as a team member with the primary care teams.

  • Communicates clearly, concisely, and accurately, creating a working climate that provides growth and job satisfaction of personnel.

  • Uses professional courtesy when communicating with patients, team members, other co-workers, and community members.

  • Contacts patients regarding lab results.

  • Identifies patients for specialized treatment services, and groups.

  • Identifies health literacy needs for patients and provides proper health education materials.

  • Upon request, provides brown bag medication review with patients.

  • Develops and/or identifies appropriate patient education materials for use with patients and families.

  • Assists with outreach activities as needed. This may include staffing health fairs, taking blood pressures, blood glucose testing.

  • Works collaboratively with other health center staff in meeting accreditation and other regulatory and/or funding requirements.

    MINIMUM JOB QUALIFICATIONS:

    Education/Certificates/Licenses:

    Required

  • Degree in nursing from an accredited nursing program

  • Current license in the State of Montana

  • BLS Certification upon hire

  • Valid driver’s license

    Experience:

    Preferred

  • Three (2) to five (5) years’ in clinical setting of primary care (RN)

  • Three (3) to five (5) years’ in clinical setting of primary care (LPN)
