Mountain Internal Medicine Care Manager in MISSOULA, Montana
The Adult Care Manager is responsible for coordinating and managing care services for adults, particularly those with chronic illnesses, disabilities, or other health-related needs. This role involves assessing clients’ needs, developing and implementing care plans, and working closely with healthcare providers, families, and community resources to ensure high-quality care and support.Come work with our awesome women-owned independent medical clinic. Work from home with occasional appointments in the clinic or client's home.Conduct comprehensive assessments of clients’ physical, emotional, and social needs.Develop and implement individualized care plans in collaboration with clients, their families, and healthcare professionals.Monitor and evaluate clients’ progress and adjust care plans as necessary.Coordination of Services:Facilitate access to medical, social, and community resources, including arranging for home care, rehabilitation services, and financial assistance.Coordinate care with various healthcare providers, including doctors, nurses, and specialists.Ensure continuity of care by overseeing transitions between different levels of care or service providers.
