Description
The Structural Heart Nurse Navigator functions on the multidisciplinary team as a clinical expert, patient advocate, educator, coordinator, and liaison for all structural heart patients and their respective family members. Responsible for ensuring all patients navigating within the structural heart program receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team. Serves as a liaison throughout the continuum of care to the patient/family as well as point contact for team members to include physicians, midlevel providers, RNs, and other department staff members. Serves as the single point of contact for referring physicians, patients, and other caregivers to provide resources and assistance with accessing clinical and supportive care services offered within the Structural Heart Program.
This position provides specialized nursing practice across the continuum of care services to patients that present with complex valvular and structural heart disease processes that require a broad spectrum of advanced medical therapy, surgical, or interventional procedures, both commercially based as well as ongoing research trial procedures. This position is involved in care from referral and initial point of contact through procedure scheduling and follow-up care. The role of the Structural Heart Nurse Navigator Lead will evolve over time to meet the needs of the patients, the cardiovascular team, and future advancement in structural heart technology.

Required Qualifications:
Upon hire: Montana Registered Nurse License
Upon hire: National Provider BLS - American Heart Association
Preferred Qualifications:
Bachelor's Degree in Nursing
Upon hire: Cardiovascular Specialty Certification
2 years Cardiac clinical nursing
