Company: Consumer Direct Care Network
Position: Registered Nurse and Licensed Practical Nurse
Schedule: Full-time available (must be flexible to work on weekends)
Location: Missoula, MT (Not remote)

As a home care nurse with the Consumer Direct Care Network, you will be an integral part of supporting our members of all ages to remain in their homes while receiving the quality skilled care they deserve. You will serve as an important part of the care team individualizing each member’s services to meet their unique needs. Nurses will provide one on one care for our members with varying caseloads dependent on member needs. Communication with supervisors, case managers, and physicians is essential in this role to ensure quality care. All cares are directed by the member’s physician, and nurses are required to adhere to agency policies governed by State and Federal Regulation.

WHAT YOU WILL DO
Work a steady schedule to support a work/personal life balance
Deliver skilled nursing care as directed by the physician’s orders and member’s personalized plan of care
Monitor and document on Member condition and report any changes to their respective physician and nursing supervisor
Develop relationships with the member’s physician and interdisciplinary team to ensure consistency and quality of skilled care
Provide education to members and families
All other duties as assigned
Graduate of an accredited school of nursing
Current RN/LPN license, valid in Montana
Current CPR certification
Valid driver’s license, automobile insurance, and reliable transportation
Must be able to successfully pass a background screening and physical therapy evaluation
