Our goal is to earn the trust of patients while providing them with the best healthcare, and improving their quality of life.Responsibilities/DutiesOur Physical Therapists examine and treat patients with physical impairments using physical modalities. The goal is to assist our patients who are physically challenged to improve mobility and function, independent self-care, and other skills necessary for functioning in daily living. The Physical Therapist will assist in all three areas (home health, hospice, and palliative).Some of the duties performed by the Physical Therapist include but are not limited to:· Creating individualized plans that can be carried out at home.· Overseeing and working with PTAs to execute exercises provided in the patients care plan.· Coordinating and communicating with other clinicians and administrative staff.· Traveling to different home care settings.· Assessing patients and monitoring progress over an episode period of 60 days.Qualifications· Must possess a valid state driver’s license.· Must possess automobile liability insurance and registration.· Must have dependable transportation kept in good working condition.· Must be able to drive an automobile in all types of weather conditions.· Must be currently licensed in the state of employment and be able to provide proof of license.· Must be able to complete a successful background check.· Travel is included. The physical therapist will be traveling all throughout the Missoula/Ravalli County.
