North West Home Care, Inc. Program Manager in Missoula, Montana

The Program Manager is a licensed professional who works under the general supervision of the Company President, a licensed RN. When providing or coordinating client nursing services with caregiver services, operate within the scope of physician’s orders and/or case management and company duty guides. The goal of home services is to provide clients with the support necessary to remain in his/her own home and be as independent as possible while maintaining the highest quality of life possible.

Qualifications:

Graduate of an accredited school of nursing
Current Montana or Compact nursing license, free of disciplinary action
Valid MT Driver’s license
One or more years supervisory experience, preferred
Technology proficiency: mobile phone applications, MS Office™ products, email, and database software
Information gathering and problem-solving abilities
Effective communication skills including active listening, providing constructive feedback and customer service skills
Organizational / time management skills
Working knowledge of State CFC/PAS program guidelines, Medicaid Waiver, VA, Private Pay, and Skilled Nursing authorizations within 3 months of employment

Implements agency policy and procedures, state and other home care service regulatory requirements
Responds to client referrals, evaluates care requirements and conducts environmental assessments
Supervises clients’ caregivers in accordance with company expectations and service requirements
Engages in client problem resolution and liaises with caregivers, office administration
Reinforces safety practices and responds to complaints, identified hazard situations and/or incidents in conjunction with Human Resources
Follows up client visits and monitors/conducts service reauthorizations/re-certifications
Determines clients’ preferred time schedules
Reviews, approves and audits electronic client care logs
Updates HR / Recruiting to target caregiver recruitment relevant to client count and service delivery
Participates in new hire orientation and other on-boarding activities as needed
Advise and orient PCA/CNA’s with individual clients and their situations
Documents and communicates performance-related situations and interventions or corrective actions
Provides performance evaluation feedback
Participates in After-hours on-call rotation
Reports client intakes and discharges; logistical or problematic caregiver situations
Advises and updates the Company President of risk situations, client services issues, addresses safety concerns
Inform clients of policy/procedures/service delivery or authorized hours changes
Participate in the recruitment and training process of Personal Care Attendants (PCAs) and Certified Nursing Assistants (CNAs) as needed.
Document problems, recommend and participate in interventions and solutions, and follow-up as necessary
Other duties may arise that further company operations or result in special project completion
