    POSITION SUMMARY

    Under direct supervision, position provides support to the residents of Group Home and the clients of Crisis House in need of 24-hour care. Assists staff to implement residents’ treatment plans.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide independent living skills training (e.g. self-care; money management; housekeeping; meal planning; foodpreparation and storage; shopping) to adults with SDMI.

  • Monitor consumer self-administration of medications

  • Assure safety of residents and clients

  • Accompany clients to physical and mental health care appointments as requested by case manager

  • Assist with development of work-readiness and pre-vocational skills (e.g. job coaching, accompany clients tovolunteer opportunities)

  • Teach and encourage community adjustment (e.g. ability to use community resources such as stores, professionalservices, recreational facilities, government agencies)

  • Provide opportunities to improve interpersonal and socialization skills, including recreational and leisureactivities

  • Perform some cleaning of residential facility

  • Perform chores necessary to assure that the residence remains clean and orderly during shift

  • Document consumer status and services provided

  • Complete paperwork consistent with agency policy and with the direction of the program manager

  • Arrive on time and participating in all required agency meetings and trainings

  • Treat coworkers, clients and community contacts with respect and professionalism

  • Comply with applicable state and federal laws, rules and agency standards;

  • Provide transportation to consumers in personal vehicle as needed

  • Maintain appropriate personal and professional boundaries with clients, families, and co-workers

  • Additional duties as assigned

    EDUCATION AND EXPERIENCE

  • High school diploma or equivalent

  • One year of progressively responsible work experience in a mental health setting

    MINIMUM QUALIFICATIONS

  • Completion of CPI (Crisis Prevention Intervention), First Aid, CPR training required

  • Ability to pass background check and driver’s license check upon offer of employment.

  • Provide proof of auto liability insurance coverage per Western’s policies.

  • Driver’s License with good driving record if required to transport clients while on work

  • High school diploma or equivalent
