SUMMARY
The Community Health Improvement Specialist will work cooperatively with other departments to promote Health Education to the SandersCounty community. Health education is a social science that draws from the biological, environmental, psychological, physical and medical sciences to promote health and prevent disease, disability and premature death by educating individuals and communities to voluntarily change their behaviors to improve their health and well-being. Health education is the development of individual, group, institutional, community and systemic strategies to improve health knowledge, attitudes, skills and behavior. The job of the Community Health Education Specialist is to positively influence the health behavior of individuals and communities.
Health education improves the health status of individuals, families, communities, states, and the nation. Health education can only be successful when integrated across a variety disciplines and delivery systems. Because of the wide variety of needs, resources available, and information, the successful candidate will take a holistic view and assist and direct the planning, collection of data and the prioritizing of goals and objectives. The Community Health Improvement Specialist will additionally work to collaborate between disparate systems, stakeholders, and providers to improve the overall health of the SandersCounty community.
The successful candidate will also be the primary lead for internal employee health improvement programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Assess individual, employee, and community needs;
Plans & implements health education strategies, interventions,
Work with department managers to administer health education strategies, interventions, and programs;
Serve as a health education resource person;
Communicate and advocate for health and health education throughout the community;
Incorporate basic bio statistical knowledge and skills into practice;
Develop and implement strategies to successfully satisfy IRS Form 990 Community Health Needs Assessment requirement;
Gathers & critically evaluates information from multiple sources & synthesizes findings. Uses information gathered to drive development of programs and strategies.
Undertakes special projects as requested by President/CEO.

MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions proportions, percentages, area, circumference, volume, incidence, and occurrence rate. Must have the ability to apply concepts of basic algebra, and geometry and have a basic understanding of statistical analysis.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit; use hands to finger, handle, or feel; and reach with hands and arms. The employee is occasionally required to stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The employee needs to be able to stand and walk for extended periods of time and be able to push a patient in a wheelchair. The employee must be able to regularly lead low to medium impact exercise programs for groups of community members and hospital staff.

EDUCATION and/or EXPERIENCE
Minimum of two years’ experience in the relevant field.
Bachelors level degree in Healthcare, Nursing, Human Performance or other field deemed beneficial to the position required.
Demonstrable communication skills and ability to communicate orally and in writing across all levels of the community we serve.
Strong cooperative leadership ability and mastery of public speaking.
Excellent customer service skills.
Ability to utilize strong coaching/consultative skills to provide guidance and resources in the achievement of CFVH goals and strategic efforts
Ability to manage multiple projects and meet multiple deadlines with excellent organizational and project management skills and strong attention to detail.
Strong computer literacy and software experience – Word, Excel, PowerPoint, Visio, Outlook, etc.
Experience in data gathering and analysis.
Wage Range: $19-$28
