POSITION OVERVIEW

Consumer Direct Care Network is seeking a self-starting, energetic and people-oriented Service Coordinator to work in a dynamic and fast-paced environment remotely from home that is based out of Sidney or Glasgow, MT. This position will require substantial travel with food and travel expenses paid for including mileage reimbursement. In this important role you will provide training and support to our members who are self-directing their services as well as managing staff that support members. The Service Coordinator will be responsible for arranging and conducting in-home visits, and are involved in day-to-day operations, referrals, intakes, and program monitoring. Successful candidates will possess strong interpersonal abilities, provide exceptional customer service, and enjoy teamwork.

WHAT YOU WILL DO

  • Answer questions about organization and provide callers with address, directions, and other information

  • Document all incoming requests for information and Member assistance

  • Submit weekly reports regarding telephone inquiries to Manager, including action(s) taken

  • Prepare and process Member and PCA charts for new intakes, annual reviews, transferred cases, and shared cases

  • Notify appropriate office staff monthly regarding annual required Medicaid reviews

  • Notify Manager of any changes in client medical condition, newly identified needs, plan of care, suspected abuse/exploitation of Member; provide notification to Manager regarding human resources issues with assistants, being mindful of the limited role as “dual employer” with Member

  • Assist in maintaining all databases and electronic files

  • Conduct search of “key” websites, such as HSD, for new program information and other information pertinent to program, such as change in codes. Keep Manager apprised of this information

  • Receive, screen, respond, and complete intake information from inquiries for Consumer Direct Care Network (CDCN) services

  • Review time sheets for accuracy and completeness and forward necessary information to time sheet auditor and the billing/accounting department on an as needed basis

  • Report to Manager any documented activity in the field that might be viewed by Medicaid as fraudulent in nature and/or against Medicaid program mandates and guidelines, and/or CDCN policies and practices

  • Act as a troubleshooter for inquiry calls regarding general program information or for Member complaints

  • Complete assessments for intakes, annual reviews, and change of needs and provide advocacy services in response to referrals and requests from the community

  • Process employee applications and route payroll information in a timely manner; uniform completeness of application; follow-up with incomplete application information; log demographic information in Member file, complete new hire report, background checks, in a timely manner

  • Create and maintain general business, and personnel files

  • Assist in maintaining and replenishing state or other CDCN required forms as needed

  • Maintain proper documentation of such activities and identify tasks/contacts for follow up, as determined by CDCN and local site policies and practices

  • Other duties as assigned

QUALIFICATIONS

  • 1-year experience in field of health care

  • Computer proficiency to include Outlook and Excel

  • Reliable transportation and valid driver’s license required

  • Maintain auto insurance according to company policy level requirement

  • Must be able to successfully pass a background screening

  • Travel required - mileage reimbursed
