State of Montana Eligibility Assistant in Warm Springs, Montana

Job Overview: This position works closely with other state and federal agencies to help patients access benefit programs to which they may be entitled due to psychiatric or physical disabilities. These include Medicare, Medicaid, Social Security, Mental Health Services Plan, and Public Assistance. Patient Case Management: 90% * Determines patient enrollment status in assistance or eligibility programs. Takes steps to maintain enrollment and related case management activities during inpatient status. * Analyzes patient status for barriers related to benefit eligibility and takes steps to enact eligible benefits for patient case. * Advocates on behalf of patients for benefit eligibility. * Represents patients for benefit appeal and administrative processes. * Develops and establishes processes for identification of patient eligibility for treatment teams. * Prioritizes cases for patient with the immediate benefit needs related to discharge and community involvement. * Monitors and evaluates status of patient benefit eligibility success. 1. Case Coordination: 10% * Educates and provides status analysis to treatment teams related to patient benefits. * Coordinates eligibility actions with families, communities, and other state or federal entities as needed. * Develops and presents educational programs to provide education and assistance to patients and staff in understanding the Medicaid, Social Security, and Medicare application and disability determination processes. * Develops materials to assist other Social Workers, Physicians, Hospital Staff, Community Workers, and patients in understanding public assistance programs. * Maintains a working knowledge of statute and processes related to benefit eligibility. Physical and Environmental Demands:The job is primarily office-based Monday through Friday. May be required to work extended work hours as needed. This position is involved in the treatment of persons who may pose unpredictable and physically aggressive behavior requiring staff intervention and possible risk of injury to staff. Employees in these positions may be expected to assist with patient crisis interventions. Knowledge, Skills and Abilities(Required for the first day of work): * Extensive knowledge and expertise related to benefit eligibility at the State and Federal level. * Knowledge of principles of social work, human behavior, contemporary social, and economic conditions, needs, and problems relating to the patient family background, and principles of federal and state legislation pertaining to social welfare community resources. * Effectively utilize knowledge and skills, and apply them to assisting with obtaining resources for patients. * Communicate effectively, verbally and in writing. * Ability to analyze, assemble, and organize information. * Ability to communicate and work with diverse organizations and patient populations. * Bachelor’s Degree in Social Work, Education, or Related Human Service field. * One year experience in mental health care related work. * One year experience working with Social Security Administration or Medicaid programs. * Other combinations of education and experience may be considered on a case-by-case basis.
