Job Overview: Social Workers provide assessment, treatment planning, and supportive individual follow up to persons who are seriously mentally ill and are hospitalized. Provides case coordination/management and development of treatment plans and reviews. Develops and implements discharge plans. Acts to ensure functional communication of needs between the patient, the hospital, the family/guardian, and community providers/resources. Direct Link to MT HB 702 Essential Duties or Responsibilities: * 1. Patient Social Assessment: 30% * Obtains relevant information to provide for patient social assessments per CMS (Center for Medicaid/Medicare Reimbursement), MSH Policy and Procedure, and Statutory and Administrative Requirements. * Acquires through research and investigation all relevant background information, including psychiatric, medical, substance abuse, and incarceration history. Includes awareness of disabilities that may need special consideration or accommodation. * Assure patient Social Assessments completed within seven (7) calendar days of patient admission. * Assure annual patient Social Assessments are completed before anniversary date of admission. * Review and address patient assessments with an interdisciplinary treatment team and to develop individualized patient treatment plans. Update patient assessments as needed to aid the team in evaluating and identifying patient strengths and limitations, determining recovery-oriented treatment options, and discharge planning needs. 2. Treatment Planning: 30% * Acts as part of interdisciplinary treatment team to formulate problems, long-term goals, short-term goals, and interventions designed to address needs of patient. Participates in clinical review and update of treatment plans within legally mandated timelines per Montana State Hospital policy and state and federal guidelines. * Documents in the medical record contacts made regarding discharge planning, contacts with family/guardian, progress made towards treatment goals, progress towards discharge as per Montana State Hospital policy, state and federal guidelines.. * Provides services to patients including, but not limited to, treatment, education, and supportive individual follow up to patients in an individual and/or group basis, using accepted therapeutic techniques and educational materials, focusing on evidence-based, recovery-oriented practice. * Serves as liaison between the patient and family/guardian relative to treatment concerns. * Identify, evaluate, and provide for Special Education Services for patients admitted to MSH who meet OPI criteria for such services. 3. Case Coordination/Management 40% * Completes written aftercare and discharge plan to include: diagnoses, patient presenting problems, patient goals, scheduled appointments, transportation, and family and community support needs as per policy and MCA codes. * Communicates with community providers patient conditional release, coordinating conditions between patient and community mental health provider to include necessary provisions. * Informs newly admitted patients of rights and grievance procedure as per policy. * Acts as appointed visitor for the court in cases of guardianship; gathers data by interviewing patient, family members, guardian volunteers; reviews patient’s residence and case record; assesses data and makes recommendations by written report to District court judges, county attorneys, and patient attorneys. * Arranges family visits and patient home visits. * Secures necessary signed release of information from patients as needed. 4. General * Maintains confidentiality related to patient information and medical records to ensure the rights of hospitalized individuals. * Performs all work tasks in a proper and safe manner following established policies, general safety rules, and safe operating procedures. * Cooperates with other staff and demonstrates respect for other employees, which contributes to a positive and efficient working environment. * Complies with Department, Division, and Unit policy and/or direction and consults as needed or required with supervisors to obtain clarification or address concerns to ensure the provision of appropriate patient care and treatment or job duties. * Complies with MSH and DPHHS policies/procedures, as well as State licensure and CMS Certification standards. * Ability to be cross trained and flex to other units as assigned. * Completes other duties as assigned. *Physical and Environmental Demands: The job is primarily office-based Monday through Friday. May be required to work extended work hours as needed. Clinical, Medical, and Nursing Services positions primarily work on treatment units and in a clinic setting. These positions are involved in the treatment of persons who pose unpredictable and physically aggressive behavior requiring staff intervention and possible risk of injury to staff. Employees in these positions may be expected to assist with patient crisis interventions. These positions are also involved in the evaluation and treatment of persons with histories of serious criminal acts. Training in hospital approved intervention techniques and Mental Health Professional Person Certification are required. Schedule:8 am to 4 pm, Monday to Friday Knowledge, Skills and Abilities: (Required for first day of work) * Knowledge of principles of social work, human behavior, contemporary social, and economic conditions, needs, and problems relating to the patient family background, and principles of federal and state legislation pertaining to social welfare community resources. * Knowledge of treatment for patients with serious mental illness, as well as chemical dependency, developmental disabilities, personality disorders, etc. * Effectively utilize knowledge and skills and apply them to practical assessment and treatment modalities for patients. * Communicate effectively, verbally and in writing. * Ability to Analyze, assemble, and organize information. inimum qualifications *(Education and Experience) :* * Bachelor’s degree from an accredited college in social work, nursing, or related field. * Two years’ experience working in a mental health or social services setting. * One year experience in a forensic mental health setting preferred. * Two years of supervisory experience preferred. * Other combinations of education and experience may be considered on a case-by-case basis.
